Terms of Agreement *

This is a legal agreement between Southpaw Solutions, LLC and the organization listed above on this form. Southpaw Solutions, LLC agrees to provide the above organization with three complimentary marketing flyers, in return for booking our team photography services. The organization can request up to 3 flyers up to one year from the date of this agreement. Should the above organization fail to comply to the terms of this agreement, said organization will be responsible for the retail price of each flyer ($50 each). Southpaw Solutions will allow one reschedule date due to inclement weather should your organization decide to have and outdoor shoot. For your reschedule date you must have an indoor space available on stand-by, should there be inclement weather a second time. Southpaw Solutions will provide each team with two team photos for coaches and the organization will receive the digital files to store in your records. The organization contact person will be mailed order forms two weeks prior to your scheduled date. Parents must pay on the day of shoot, NO EXCEPTIONS. We accept cash, money orders, and major credit cards. NO CHECKS.
